Who has parental responsibility for your child? *
Note: In England and Wales, if the parents of a child are married to each other at the time of the birth, or if they have jointly adopted a child, then they both have parental responsibility. Parents do not lose parental responsibility if they divorce, and this applies to both the resident and the non-resident parent. This is not automatically the case for unmarried parents. According to current law, a mother always has parental responsibility for her child. A father, however, has this responsibility only if he is married to the mother when the child is born or has acquired legal responsibility for his child through one of these three routes: ·1) by jointly registering the birth of the child with the mother, 2) by a parental responsibility agreement with the mother, 3) by a parental responsibility order, made by a court.

PHOTOGRAPHS AND PROMOTIONAL MATERIAL
We regularly take photographs or make video recordings of the children to document the children's achievements and the Pre-school's activities. Similarly, there are occasions when the local press may visit to record particular events or we may supply photographs as part of a press release celebrating activities at Pre-school. Please provide your consent for us to take photographs or make video recordings of your child for the following purposes:
SHARING INFORMATION WITH OTHER EARLY YEARS SETTINGS
If your child attends another Early Years setting such as another pre-school, nursery or childminder then we are encouraged to share information on your child's learning and development with the other setting(s).
I hereby consent for my child to take up a place at this Preschool, according to the terms and conditions set out in its Constitution and Policies and Procedures. I have understood the expectations and obligations relating to both myself and the Preschool, and agree to abide by them.
I understand that Freshford Preschool is required to follow Bath and North East Somerset's Safeguarding Procedures. The Preschool has a duty to contact Social Services if the Preschool has concerns for a child's welfare. The Preschool will normally contact parents/carers prior to speaking to Social Services, unless it is felt that this action would significantly increase the risk to the child.
I agree to the terms of the Preschool's Fees policy and understand that persistent late or non-payment of fees will jeopardise my child’s continued attendance at the Preschool. I confirm that the information given above is correct, and I agree to contact the Presschool Business Manager as soon as any of the details change.
